CONTINUUM OF CARE

 

The Continuum of Care (CoC)

Program is designed to promote community-wide commitment to the goal of ending homelessness; provide funding for efforts by nonprofit providers, and State and local governments to quickly rehouse homeless individuals and families while minimizing the trauma and dislocation caused to homeless individuals, families, and communities by homelessness; promote access to and effect utilization of mainstream programs by homeless individuals and families, and optimize self-sufficiency among individuals and families experiencing homelessness.

The TN Valley Continuum of Care is comprised of advocates, health care, mental health, substance abuse, homeless providers, food pantries, affordable housing developers, legal, disability advocates, LGBTQ advocates, youth, veterans, families, police, and so many more.

Homeless Management Information System

A Homeless Management Information System (HMIS) is a local information technology system used to collect client-level data and data on providing housing and services to homeless individuals, families, and persons at risk of homelessness. Tennessee Valley Continuum of Care (CoC) is responsible for data collection, management, and adherence to HUD reporting standards. For more information, please click the button below.
